Abstract
Background: The Interventional Radiology (IR) involves procedures with the potential for complications. Human error is an inevitable cause of patient harm. Safety checks are a simple method of reducing the risk of harm using a periprocedural routine to reduce the likelihood of error. According to Royal college of Radiologist â??Checklists for radiological procedures are a simple and effective way to help and prevent incidents and incorrect procedures.â? Aim: The aim of the audit evaluates the adherence to use and complete the WHO Checklist for procedures done in the interventional radiology department. Methodology: First cycle: Retrospective data collection in September 2022 for 200 procedures done between September 2021- September 2022. Issues identified:
â?¢ Incomplete checklist.
â?¢ No electronic record of checklists Data analysis:
â?¢ Percentage of undocumented WHO Checklist: 8%
â?¢ Percentage of incomplete checklist : 26%
â?¢ Percentage of undocumented and incomplete checklists: 34% Implementing a change:
â?¢ Local presentation to Interventional radiology staff about the importance of checklist.
Second cycle: Retrospective data collection after 2 months for procedures done between September 2022 to November 2022. Data analysis:
â?¢ Percentage of undocumented WHO Checklist: 9%.
â?¢ Percentage of incomplete checklist: 11%.
â?¢ Percentage of undocumented and incomplete checklists: 20%.
Conclusion:
â?¢ Almost Unchanged percentage of undocumented Checklist.
â?¢ Percentage of completing the checklist has improved from 74% to 89%.
â?¢ Percentage of documenting and completing the checklist has improved from 66% to 80%.
