Intramuscular hemangioma of the orofacial region: Literature review and a case report

Abstract
Abstract: Intramuscular hemangiomas are unique vascular tumors. They are benign in nature, most commonly occurring in the trunk of extremities. In the head and neck region, they are present and constitute less than 1% of body hemangiomas. We report a case of a 21-year-old Sudanese male patient who presented to our clinic with a fluctuant swelling of the left cheek that was gradually increasing in size over the past 7 months. Preoperative magnetic resonance angiography (MRA) showed a vascular tumor on the left cheek adjacent to the buccinator and masseter muscles. The lesion was injected with hot saline every two weeks for more than two months till the tumor became fibrosed and firm. Surgical excision under general anesthesia was performed via an extra-oral approach. The external carotid artery was ligated. Quilting around the tumor was performed, and the mass was excised safely and uneventfully. In conclusion, Intramuscular hemangiomas should be considered in the differential diagnosis of musculoskeletal pain associated with fluctuant soft tissue swellings. Hot saline is an effective sclerosing agent, that can be used in the management of soft tissue hemangiomas safely and effectively. Both quilting and ligation of the external carotid artery are helpful and effective in decreasing bleeding when excising such tumors
