Needles to state, correct diagnosis is the fundamental basis for the appropriate treatment and management of a patient
in any disease. Orthopaedic surgery for bone tumors is a highly specialised area & is performed usually in specialised
centres. The type of specimens received in orthopaedic oncology are needle biopsies, curetted samples, open biopsies, wide
resections, amputation specimens, disarticulations & specimens for evaluation of neoadjuvant chemotherapy response. But
needle biopsy is the most commonly received specimen in most of the pathology laboratories. Because of the overlap of
histological features between Reactive and Reparative Lesions, Benign & Malignant Tumors, sharing of age & location of the
tumor correct diagnosis becomes a challenging task. Therefore it is very essential to be aware about the pitfalls of the core
biopsies & differential diagnosis of Bone Tumors. The biopsy can be Representative or Representative but Inadequate or Non-
Representative. The problem areas are usually Giant lesions of bone (Reparative, benign & malignant), Round Cell Tumors of
bone, Fibrous lesions with bone & tumors with presence of cartilage. Various methods & techniques are applied to arrive at
definitive diagnosis. With the significant improvement in the prognosis it is obligatory on the part of the pathologist to make
correct diagnosis and convey the same to the clinician for better patient care and management. It is important to integrate
modern Molecular methods, Radiology & Morphological pathology to the better understanding of tumors, risk stratification
& better treatment of the patient to prolong & improve the quality of life.
