A new, safe and reproducible procedure to improve soft tissue augmentation outcomes

Abstract
Background: In the field of fat graft, the combination of autologous in vitro-expanded ADSC with fat for clinical applications overcomes some of the problems of fat graft alone, as variable resorption and survival rates, but there is still a high technical variability that could negatively affect the clinical outcomes. A standardized and reproducible technique to in vitro expanded ADSC, that can increase the viability of the transplanted cells, is required. Method: In this frame, we decided to explore injection of in vitro expanded autologous ADSC in association to autologous fat for breast augmentation and face rejuvenation. ADSC were isolated from autologous fat and expanded according to GMP procedures, using a cell growth medium supplemented with pooled human platelet lysate, which provided a significantly higher proliferation rate compared to FBS. Before release, cells were tested for sterility and biological safety. In vitro and in vivo experiments were performed according to the European Pharmacopoeia and no signs of tumorigenesis were detected, suggesting the biological safety of expanded ADSC. Cells were administered subcutaneously in association with fat to 6 patients for breast augmentation and 5 patients for facial rejuvenation. Evaluation of the treated areas was performed prior and after treatment and 4 months after by means of MRI and bilateral mammography, in compliance with an established follow-up protocol. Results: Follow-up reports highlight an increase in the volume of adipose tissue in treated areas, due to ADSC-mediated enhancement of fat graft viability and suggest that the ratio ADSC/fat is critical for graft retention. No adverse events have been recorded. Conclusion: Our study confirms that fat graft enriched with in vitro expanded autologous ADSC is a safe and efficient approach for soft tissues augmentation, since it reduces fat graft resorption rate and ensures durable results. Moreover, thanks to our highly reproducible protocol, in vitro expansion of ADSC increases the number of cells to the quantities required to achieve therapeutic results and overcomes the limited amount of stem cells that can be harvested from a single patient in a single surgery.

Born May 29, 1961 Italy, Giuseppe Mucci started off his journey studying at thernUniversity of Urbino \"Carlo Bo\" in the Faculty of Medicine and earned a degree inrnPhysical Education later he specialized in psycho-motility. He has been always interested in all innovative approaches in the medical field so that in his business he could positively improve the health of humanity in respect of human physiology, avoiding chemistry and invasive surgery. He has covered important roles in introducing the most advanced medical informatics in medical nutrition therapy and body composition, as well as having transferred the innovation of the aerospace technology for the early detection of melanoma by video-dermoscopy to optical probe. After a solid introduction into the world of medicine, Giuseppe then lifted his career by founding his company in 1999, operating in the sector of equipment andbiotechnology. Giuseppe Mucci has always considered knowledge as a cornerstone in the development of his company and of the surrounding world, for this reason in 2005 he launched the Bioscience Foundation (UK) dedicated to knowledge sharing, content management, T-learning and peer-to-peer learning in medical science. In the following year he established Bioscience Institute in San Marino Italy, built as a Cell Factory taking advantage of the latest technological solutions available on the market at the time.
