By combining physical health, mental health, and social services, these models address the multifaceted needs of patients, improving health outcomes and patient satisfaction while reducing healthcare costs. This article explores the definition and components of integrated care models, their benefits and challenges, examples of successful implementation, and future directions in the evolution of healthcare delivery. Emphasizing collaboration among healthcare providers and a focus on holistic patient care, integrated care models are poised to transform the landscape of health services
