Community-based nursing is a vital and evolving healthcare approach that places emphasis on providing comprehensive, patient-centered care within the context of a community setting. This nursing practice model focuses on promoting health, preventing disease, and managing chronic conditions by addressing the unique needs and challenges of individuals and populations in their own communities.
