Abstract

The corneal endothelium plays a pivotal role in maintaining the transparency and clarity of the cornea, essential for optimal visual acuity. This review delves into the intricate functions of the corneal endothelium, exploring its physiology, regulatory mechanisms, and the challenges it faces in maintaining corneal homeostasis. Special emphasis is placed on elucidating the mysteries surrounding the regeneration and repair processes of the corneal endothelium, shedding light on the cellular dynamics involved. The second part of this abstract shifts focus to corneal endothelial transplantation, a critical intervention for restoring vision in patients with endothelial dysfunction. A comprehensive overview of available transplantation techniques, including Descemet's Stripping Endothelial Keratoplasty (DSEK) and Descemet's Membrane Endothelial Keratoplasty (DMEK), is provided. The abstract highlights recent advancements and emerging technologies in the field, aiming to improve transplantation outcomes and expand donor pool accessibility. Introduction

The human cornea, a remarkable and intricately organized structure, serves as the transparent window through which light enters the eye, enabling vision. At the heart of maintaining its crystal-clear clarity lies the corneal endothelium, a monolayer of cells positioned at the posterior surface of the cornea. These endothelial cells play a pivotal role in regulating fluid balance, preventing edema, and preserving the transparency required for optimal visual acuity [1,2]. This review, titled "Guardians of Clarity: Unraveling the Mysteries of Corneal Endothelium Function and Transplantation," embarks on a journey to unravel the complexities surrounding the physiological functions of the corneal endothelium. Understanding the mechanisms that govern corneal homeostasis and the cellular dynamics involved in its maintenance is paramount for comprehending ocular health and addressing vision-threatening conditions associated with endothelial dysfunction [3,4]. The first segment of this exploration delves into the intricate dance of cellular processes that underlie the regeneration and repair of the corneal endothelium. As guardians of clarity, these cells face continuous challenges, and deciphering the mysteries of their resilience and regenerative capacity is essential for advancing therapeutic strategies [5,6]. Transitioning from the physiological nuances, the second part of this review shifts focus to the realm of corneal endothelial transplantation. Highlighting techniques such as Descemet's Stripping Endothelial Keratoplasty (DSEK) and Descemet's Membrane Endothelial Keratoplasty (DMEK), this section provides a comprehensive overview of current transplantation methodologies [7,8]. Furthermore, it explores recent advancements and emerging technologies poised to revolutionize the landscape of corneal transplantation, aiming to improve outcomes and make this vital intervention more accessible. Material and Methods
